    Job Summary
    Provides medical oversight to the Community Day Services and Residential Program under the direction of the Executive Director.

    Responsibilities and Duties

    • Serves as a member of the Intake Committee. Responsible for securing information related to health and medical histories, diagnosis and treatment, and preparing medical/social history.
    • Responsible for all Nursing assessments for people supported, including Self-Administration Medication Assessment (SAMA), Health Risk Screening Tool (HRST), change of condition assessments and medical evaluations. 
    • Makes recommendations on for corrective action of medication administration authorized staff. 
    • Reports to or consults with the physician as needed about medical, physical, social, emotional, and other concerns pertaining to the nursing care/developmental needs of the person.
    • Conducts home visits as appropriate to monitor health needs and to evaluate medication administration authorized staff through the Competency Based Training Assessment (CBTA) process.
    • Maintains medical records for all people supported.
    • Works in conjunction with the LPN to ensure all medical needs are met.
    • Must be licensed as a registered professional nurse or advanced practice nurse in Illinois and successfully completed the DHS Nurse-Trainer Training Program.
    • Abides by the Mental Health and Developmental Disabilities Administrative Act and the Illinois Nursing and Advanced Practice Nursing Act.
    • Serves on Behavior Support Committee and Human Rights Committee of Pathway Services Unlimited, Inc.
    • Conducts Medication Administration Training for staff including Medication Administration Reauthorization, Medication Administration De-authorization and Med Pass Observations (MPO).
    • Works collaboratively with nursing consultants and/or Station MD personnel to achieve the best medical outcome for people supported.
    • Responsible for Nursing on-call rotation as required by Administrative Rule 116.
    • Serves as a member of the Care Team, ensuring all appropriate medical trainings, medication audits and all relevant Residential and Community Day Services meetings are attended.
    • Reviews the Medication Administration Record (MAR) on a regular basis for quality assurance, as it pertains to accuracy in conjunction with the Physician Order Sheet (POS) and the medication label, medication errors and appropriate documentation by medication administration authorized staff.
    • May be required to administer medications as needed.  
    • Completes all annual training requirements.
    • May be required to assist during emergency situations.
    • Performs all other duties as assigned.
    Skills and Qualifications
    Minimum Education/Experience
    Verifiable documentation of education. Certification and/or degree that meets requirements for Registered Nurse Trainer (RNT) as defined by the Illinois Department of Human Services. Registered Nurse certification.  Possess two (2) years of clinical registered professional nursing experience within the last five (5) years, at least one of which shall preferably have been in developmental disabilities. Must have a valid Illinois driver’s license and insurable driving record.

    Language and Skills
    This is a professional position which requires excellent verbal and written communication skills. Successful completion of the Short Test of Functional Health Literacy in Adults (S-TOFHLA) test, testing ability to read.  Ability to prioritize responsibilities and adapt to the changing needs of the Nursing department is required.  Ability to interact and communicate with people supported by our Agency, employees, parents/guardians, State/Federal staff members, and other supervisory/management personnel.  Able to appropriately utilize available computer programs and systems is required.

    Physical Demands
    Must be able to complete all physical requirements of the position with or without a reasonable accommodation.  Successful completion of Essential Functioning Test verifying ability to lift at least seventy-five (75) pounds.  May have a flexible varied schedule with nights and/or weekends required.

    $68,744 - $71,244 per year
